Sweatshirts - $25 Sweatpants - $20 T-Shirts - $5-$15 Glitter Lanyards - $8 St. Vincent SPORTS PHYSICAL NIGHTSUNDAY, MAY 6, 2012--St. Vincent will be performing our Annual School Physicals. Cost is $20 with $10 from each physical going back to the school. They will be held at the St. Vincent Sports Performance Center at 8227 NorthWest Blvd. Suite 160 from 3:00-6:00 p.m. If you are unable to make that date, there will also be another opportunity on June 10, 2012 at St. Vincent Carmel Hospital at 13500 N. Meridian, Entrance 5. You can visit www.ihsaa.org/forms/PhysicalForm.pdf to print off the IHSAA Pre-participation physical evaluation form and bring it with you.
